1. NAME (Person, Company or Organization) Write in the exact name of the individual, business, government agency, bank, professional firm or organization that will be listed on the account. 2. CONTACT PERSON: If the account is not an individual account list the name of the person in the business or organization to be contacted by us when we have questions about the account and to whom we will direct our mail and billing to. 3. ADDRESS: Be sure to write the address very clearly as
this is used for billing and identification purposes.
All telephone numbers should be written with the country code in brackets, the city or area code (where required) in parenthesis, and the local number. The international access code varies from country to country and does not have to be shown on the CUSTOMER ORDER FORM. EXAMPLE: An international direct dial phone call placed from the United States to Madrid, Spain would be as follows: 011 [34] (1) 577-xxxx where:An international direct dial phone call placed from another country to Washington, DC, United States would be as follows: international access code + [1] (202) 939-xxxx where:5. PAYMENT INFORMATION: Payment by international credit card is required in most cases. Large companies can apply for open account with or without initial deposit and pay bills for service monthly (trade references and letter of credit may be required to secure account). To prevent credit card fraud and for the protection of GlobalTel and the customer, all customer order forms should be signed, and accompanied by a copy of the both sides of the credit card or a copy of a recent billing statement from the bank issuing the card. A copy of a PHOTO ID (Passport or Driver License is also required. When copying the credit card and photo ID, set the copier for making light copies (as opposed to normal or dark). Do not send credit card numbers by email, send them by fax. 6. PHONE NUMBER TO BE CONNECTED: Clients may have two (2) access numbers without charge. Additional access numbers cost $1.00 each per month and the charge is added to the customers monthly bill. A client should list all home and office telephones, faxes and cellular telephones which will be used for making international calls. 7. ACCESS NUMBER: Access numbers will be assigned by GlobalTel to each phone number listed. Access numbers are special Direct Inverse Dialing phone numbers for GlobalTel's switching computers. They have seven (7) DIGITS - a three digit area code and a seven digit local number: for example: (305) 999-9999. 8. SECURITY CODE (PIN number), and ACCOUNT NUMBER: Customer can choose 4-digit PIN number, and GlobalTel will supply ACCOUNT NUMBER. The PIN NUMBER and ACCOUNT CODE are not used in basic callback operation. They are required in order to change the callback number when traveling, or for switching the callback number from your office to home or cellular phone when you are not at your callback number. These numbers are also necessary to access online bills and use this service as calling card for calls from USA and Canada. 10: LANGUAGE CHOICE FOR VOICE PROMPTS: GlobalTel offers voice prompts in English, Spanish and Portuguese, Russian and other languages as shown on current CUSTOMER ORDER FORMS. Unless a choice is made, English will be used. 11. COMMENTS: Here write any information which may be helpful
to us in approving or connecting the customer. For example, if several
languages are spoken, or if the customer travels frequently, or if there
is a speech impairment.
